Starting on June 1st, 2024, we are offering you a chance to win one of these three attractive prizes for doing nothing more than filling out an entry with every case that you send to our lab. The entry period will run through August 31st, 2024. Each entry will include space for the doctor’s name and a staff member’s name. Both names on the entry form will have their choice of one of these prizes! Every case you send is eligible. For example, if you send 4 cases in a box, you can submit 4 entries. We appreciate your business and want to wish you luck in winning one of these fabulous prizes. Entry forms will be included in all returned cases.
Terms and Conditions
Promotion Period: The Burdette Dental Lab Customer Appreciation promotion begins on June 1st, 2024, and ends on August 31st, 2024 (“Promotion Period”).
Eligibility: The promotion is open to all existing clients of Burdette Dental Lab who submit cases during the Promotion Period.
Entry: Clients will automatically receive one entry for each case they submit to Burdette Dental Lab during the Promotion Period. Entry forms will be included in all returned cases. Clients must complete the entry form with the doctor’s name and a staff member’s name to be eligible.
Prize Selection: At the end of the Promotion Period, three prize winners will be randomly selected from all eligible entries received. Each winner will have their choice of one of the following prizes:
- $1000 Cold Hard Cash
- APPLE Airpods Max & iWatch
- Blackstone Pellet Grill Smoker and Omnivore Griddle Combo – 2-Burner Liquid Propane Flat Top Grill
Prize Claim: Winners will be notified via the contact information provided on the entry form. Winners must respond within seven (7) days of notification to claim their prize. Failure to respond within this timeframe may result in disqualification, and an alternate winner may be selected.
Prize Delivery: Prizes will be delivered to the address provided by the winners within 30 days of prize claim verification.

No purchase necessary to enter or win.
Prizes are non-transferable and cannot be exchanged for cash or other alternatives.
Burdette Dental Lab reserves the right to substitute a prize of equal or greater value in the event of unavailability.
